PLEDGE SHEET - this is the form you will use to record pledged amounts (offline donations) from sponosors. You will submit this form, with your name and address, along with the personal checks you receive to One Mission, P.O. Box 600157, Newtonville, MA 02460. . Off-line donations will be added to your fundraising page totals within 7-10 business days of receipt.
CASH DONATION FORM - the form you will use to record cash donations received from sponsors. One Mission does not accept cash donations. You will need to write one personal check for the total amount of cash donations received and submit with this form to One Mission, P.O. Box 600157, Newtonville, MA 02460. Off-line donations will be added to your fundraising page totals within 7-10 business days of receipt.
